Stroke Rehab provides educational content, services, and products for creating, managing, and modifying the lifestyles and environments of adults or older adults to increase safety and independence and allow them to age successfully at home. (“Services”). Services include the home safety assessments, fall prevention programs, health coaching for weight loss, and occupational therapy services designed to help you (in-person or online), create, maintain, or modify your lifestyle or home to create healthier and safer lives.

Access to certain portions of the Site are limited to registered users of our Services. As part of our registration process, you must provide us with certain personally identifiable information. This information is necessary so that we can verify your identity, and make the full use of the Services we offer through the use of our Site. In addition to, you may be required to provide a credit, debit, or charge card number, or other payment information, as well as your name, telephone number(s), email, and/or street address, and other personally identifiable information, which will be maintained and used by us as permitted by these Terms and the Privacy Policy.
